Session 2: Patient Engagement

Digital Health Solutions that Patients Want

Glooko’s virtual roundtable series, New Frontiers in Connected Care, brings thought leaders from the healthcare industry together to share their thoughts and insights on innovations in digital health, and the impact technology can have on improving chronic care management.

In the second part of our continuing series, our panel focuses on patient engagement. Our practicing clinicians discuss how best to educate, coach, and support patients to better manage diabetes and related chronic conditions. There are a wealth of new technologies and programs, all striving to improve clinical outcomes, provide a better patient experience, and be super convenient to use. This webinar benefits anyone trying to make sense of these various options in order to implement something that patients actually want to use.

Panel members cover topics including:

  • The biggest challenges practicing clinicians face related to coaching and supporting patients with chronic conditions

  • The best and most promising tools to help improve patient engagement and adherence

  • The shift from periodic in-person visits to more frequent, remote updates and touchpoints

  • How to address structural issues around health equity and social determinants of health
