Evidence:

A VIRTUAL DIABETES CARE MODEL TO IMPROVE SELF-MANAGEMENT: AN EMPLOYEE WELLNESS PROGRAM CASE STUDY

Glooko and Cecelia Health partnered with a large Midwestern hospital system to deliver a virtual care diabetes support program to its employee members and their dependents.

Participants (n=126) received one-on-one diabetes education and health coaching from a Cecelia Health CDCES and full-access to the Glooko user app to track and monitor food intake, medication and activity.

Results suggested strong patient engagement and motivation to participate. 12-week results revealed clinically meaningful reductions in A1C and above average time-in-range.
